“My X-ray says bone on bone. Does that mean I need surgery?”

Not necessarily — and the research backs this up. A 2008 study by Bedson & Croft in BMC Musculoskeletal Disorders found a surprisingly weak correlation between how severe an X-ray looks and how much pain a patient actually experiences.

Some people with severe “bone on bone” findings on imaging have mild pain and function well. Others with moderate X-ray changes are in significant distress. This is why surgeons consider your symptoms, function, and quality of life — not just the image — when recommending surgery.
